Did you provide us with your insurance information?

So many services and products require authorization by medical insurance.  Providing us with your insurance information BEFORE your appointment allows us to see what products and services your insurance will cover before we see you at your appointment.  Again, this will allow us to concentrate our time on you and your foot issue instead of waiting on the phone with your insurance company during your appointment.

Please provide your insurance information and remember to include:

  • Name of Insurance
  • Member ID/Policy Number
  • Group Number
  • Member Service Phone Number or Customer Service Phone Number

Our website provides a comprehensive list of conditions and treatments.  Feel free to browse this website and familiarize yourself with the information prior to your appointment.  Our website answers many questions about podiatry, various foot conditions and our office policies and procedures.

If you have any questions regarding your upcoming appointment, please call our office at 816-943-1111.  Our office staff is available 8-5 Monday-Thursday and 8-4 on Friday.

Thanks, and we look forward to seeing you!
